Ninja TAG Active Ipswich (Risk Acknowledgment Disclaimer)
Ninja TAG Active is a physically challenging assault course style arena and by participating you understand that the activity is high risk and failure to follow the safety rules could result in serious injury.
By completing and signing this agreement you are consenting to the terms of this acknowledgement and disclaimer and you are agreeing to your own participation in the events within Ninja TAG Active Ipswich. This document is valid for 12 months from completion of this form.
Ninja TAG Active | Prerequisites for entry & participation.
- I am 18 years old or over.
- I acknowledge that the maximum permitted weight for participation is 20.5 stone (130kg). You may be weighed if you appear to approach this weight.
- I certify that to the best of my knowledge I am not pregnant.
- I agree that I will participate in Ninja TAG Active in accordance with the ‘Ninja TAG Active Arena Rules’ and ‘Ninja TAG Active Dress Code’ made available to me prior to participation and at / within the arena (which can also be viewed at https://www.superbowluk.co.uk/ipswich/ninja-tag-active) in addition to
the advice given in the pre-participation safety briefing, together with any oral instructions or advice given to me prior to or during the session. - I acknowledge that I am responsible for my own safety (and the safety of my possessions) whilst in Ninja TAG Active.
- I agree that I will only attempt events that I feel are within my physical and mental ability level.
- I certify to the best of my knowledge that I am in good physical condition and I do not have any known medical condition(s) which may have the effect of making it more likely that I will be involved in an incident which could result in injury to myself or others.
- I certify that I have no pre-existing medical conditions which, could through participation in Ninja TAG Active, result in injury to myself or damage to my health.
- I agree that I do not knowingly suffer from vertigo or claustrophobia.
- I agree that at the time of participation I am not under the influence of alcohol; and/or any illicit drugs; and/or prescribed medications that might impair my ability to undertake the activity safely.
- I agree that Ninja TAG Active will not have any liability to me for any loss or damage of clothing or other personal items resulting from participation in Ninja TAG Active.
I have had sufficient opportunity to read this entire document. I have read and understood it, and I agree to be bound by its terms.
